Hi David. This guide tells you exactly what to do, in order. You do not need to know anything about taxes to follow it. Every step is explained in simple words. If you get stuck on anything, call or text Marquecia at (424) 470-7407.
We built you a special website that asks you questions about every year from 2020 all the way through 2025. It is six years, one tab at a time. This is the most important thing you can do right now.
When the page opens, you will see tabs at the top that say 2020, 2021, 2022, 2023, 2024, 2025. Start at 2020 and work your way across to 2025. Click "Save [Year]" before moving to the next tab.
- Which vehicle did you drive for that job? (Harley, BMW, or Truck — pick one per job)
- Your cell phone bill — the total amount you pay each year, and what percent you use it for work
- Equipment you bought — any tools, devices, or supplies you purchased for work. What it was, how much it cost, where you bought it
- Uniforms or work gear — clothing you only wear for work, not regular clothes. Work boots, safety vests, branded shirts
- Car insurance — how much you pay each year to insure each vehicle
- Commissions you paid anyone — did you pay someone to help you get a job or make a sale? We need their name and how much
At the very end of the 2025 tab, click the big button that says "Submit All Years." That sends everything to Kathy automatically.
We need your bank statements from USC Credit Union for multiple years. A bank statement is the monthly PDF that shows all the money that went in and out of your account. You can download them from your online banking app or website.

We are also missing all 12 months of 2022, all 12 months of 2023, and all 12 months of 2024. That is 36 statements total. We know that sounds like a lot — but you can download them all at once from the USC Credit Union app under "Statements."

You do not need a Dropbox account to upload. Just click the link, choose your files, and hit upload. That is it. Kathy gets them automatically.

These are the official tax papers that companies and employers send you. You need to gather these and upload them to the same Dropbox folder. Here is the full list:
- W-2 from Fiber Optic Solutions — your employer for December 2025. They mail this by January 31. If you have not received it, call HR at Fiber Optic Solutions and ask for your W-2.
- Form 1098 (Mortgage Interest) — your lender mails this every January. It shows how much mortgage interest you paid on your Victorville home. If you can't find it, call your mortgage lender and ask for your 2025 Form 1098.
- TVG Network Win/Loss Statement — log into your TVG account, go to Account Settings or Tax Documents, and download your 2025 statement.
- Casino Win/Loss Statement — go to the players club desk at each casino you visited in 2025 and ask for your annual win/loss statement. This is free.
- Robinhood / Investment 1099 — open the Robinhood app, go to Account → Documents → Tax Documents, and download your 2025 Consolidated 1099. Usually available by February 15.
- Any 1099-K forms — Cash App, PayPal, or Zelle may mail or email you a 1099-K if you received over $600 through their platform. Check your email and app notifications.
- Any other 1099s — if any company sent you a tax form in January or February 2026, include it. When in doubt, upload it and let Kathy decide.
- Form 1095-A — ONLY if you had Covered California health insurance in 2025. Covered CA mails this in January. If you had insurance through your job or paid for it yourself, you do not need this form.
There are four documents that the IRS and the California FTB require you to sign with a real pen on real paper. They do not accept electronic signatures or typed names for these forms. This is a legal requirement.
Here are the four documents and what each one does:
This gives Kathy permission to look at your IRS records. Without this, the IRS will not talk to Kathy about your account at all.
This gives Kathy full permission to represent you with California — make calls, respond to notices, and challenge the penalties on your behalf.
This officially removes your old tax preparer (Obaseki) from your IRS account so he can no longer access or interfere with your case.
This is your agreement with Purnell Tax Solutions. It lists all the work Kathy will do and the fees. You also select your payment option on this form.
Step B — Sign: Sign every signature line with a blue or black pen. Write the date next to your signature. Do not type or stamp — it must be handwritten.
Step C — Scan: Use your phone to scan the signed pages. The free iPhone app "Notes" can scan documents, or use the Adobe Scan app (free on Android and iPhone).
Step D — Upload: Upload the scanned PDFs to the Dropbox folder below. Name them clearly, like "Signed Form 8821" so Kathy can find them easily.
After Kathy uploads your signed FTB Form 3520-PIT to your California account, the California FTB (state tax agency) will mail a letter to your home address. Inside the letter is a one-time authorization code.
The letter will ask you to go to the MyFTB website and enter the code. This confirms that you really did give Kathy permission to represent you. Without this step, the FTB will not allow Kathy to act on your account.

4. Enter the authorization code from the letter

6. Text Marquecia at (424) 470-7407 to let her know it is done
- 🚫Do NOT call the IRS or FTB on your own. If you try to talk to them without Kathy, you could accidentally say something that makes the situation worse. Let Kathy handle all contact.
- 📬Forward every notice you receive. If you get any letter from the IRS or the California FTB, take a picture of it and send it to Kathy that same day. Do not ignore or throw away tax mail.
- 💳Do not agree to any payment amount. If the IRS or FTB calls you, be polite, take their name and number, and call Marquecia immediately at (424) 470-7407. Do not agree to pay any amount without Kathy's guidance.
- ✅Stay in contact. If Kathy texts or calls you, respond the same day. Delays on your end create delays in filing, which keeps your case open longer.
